can you check forum – configuration – storage locations and make sure you have green marks for avatars so you actually have a directory with correct permissions…

fiz, more details please… are you really using version 4.1.x???

check for js errors in the console when uploading… also try disabling loading of forum js in footer… its in forum – wp integration – page and permalink…
