Strange thing is that the plugin seemed to create all the other folders it needed just not the avatars and smileys folders (directories):

you are going to have to contact your host. being able to create directories on your server really doesnt have anything to do with simple:press

After reading some long support threads about people with a similar problem, I reflected on the fact that it was at least creating some directories in the blogs.dir file. So I just uninstalled and reinstalled the plugin and this time it created all the spf directories. So for now, I'm done. Thanks for letting me try your patience.
