I had to reinstall the plugin. I did a delete of the prior installation and uploaded the most recent version. After the upload and activation, the "forum" menu and all submenus are present, but the "install" sequence does not occur. I have tried deleting and re-installing/activating three times with the same result. Can you either offer 1) a way to activate the "install" sequence; or 2) give me instructions for accomplishing the same thing manually?

Yes – if you want to delete everything then you also need to remove tables and database records. If the data is still there SP assumes it is already installed – which is technically correct.
So – if you want to completely remove and then re-install – use the uninstall option and THEN deactivate the plugin.
This is explained in the wiki – on the Uninstall panel of the Toolbox – and there should also be an uninstall option on the plugin listing for SP.
