Oh, and: in the final file and in your example above lines 2 and 4 are exactly the same. Is that correct?

No keep it as you changed it as that is correct.
We should be issuing an update in the next week (or so) which will include both of these fixes.

So I put this exactly as you wrote earlier? With the TWO lines beginning with "<input type="checkbox"
