Steve will hopefully be able to confirm when he is around later but I seem to recall that Simple:Press 5.4.1 required a later WordPress than 3.7.1. Were we not into WP 3.9 by that time?

simple press 5.4.1 requires wp 3.8 or later... would have been on the download page and still in the main plugin file (sp-control.php) as required...
is that your issue? hard to tell... but you have certainly missed some critical security updates in wp (and even on in simple press)...
additionally, you didnt specify which version of the file uploader plugin you were using (last modified date doesnt really help), so could have additional compatibility issues...
