Support Forum

Hi all -

I have run into three issues (these are the ones I have discovered so far) with our site I would greatly appreciate help with:

1. When creating a new forum, the screen incorrectly populates the available options.

2. Another user was able to create two subforums inside a non-existent forum that shows up on the front end but not the backend (shows on the frontend w/o a title). They intended to create these as forums. I believe this is a result of #1 not functioning properly and confusing the user. How can I remove this "empty" forum?
   
3. I added three new forums and the "+" for adding topics does not work. Any ideas as to why?
