A user emailed to say that he has this problem. My own account is fine but when I log in as him the button is missing for me too. Any idea what's wrong?

see what usergroups he is a member of...  view his sp profile and you can see usergroup memberships as well as view his accesses for any forums...

I have had a look at his profile. He doesn't have permission to create or reply to posts. How do I change that?

add him to a usergroup that does have that permission... or modify the permission set applied to his usergroup...

The user concerned needs to be added to the members user group but he doesn't appear in the list of eligible members on the left hand side. How do I make him eligible so I can add him to the user group?

What do you mean by not eligible?? do you mean he doesnt show up in the list of users when you try to add one? did you filter the list for his display name? only partial lists are shown at a time... if you did mean, he doesnt show in list, two reasons for that... one, he is an admin (doesnt seem like the case)... two, he is already in the usergroup (you can check that in same location - view members)...

Yes, he's already in a user group, the default guest user group without permission to create or reply to posts. I want to move him into the default members user group. How do I do that?

Incidentally, the vast majority of the site's 8,000 WP users appear to be in the guest user group rather than the members user group where I want them. What governs which user group they are added to when a WP user is created and how can I move the current guest user group members to the members user group?

Under the User Groups menu section is the admin panel where you can map users to user groups. This s based upon mapping users in WordPress roles to Simple:Press user groups. You can also re-map all members on this panel should it be necessary.

Remember that the name of a user group has to relevance to how it works. You can name it whatever you want. The fact that one is called 'guests' is just something we set up by default as a convenience. The real power of user groups is how they are joined with permission sets and then that combination assigned to forums.

So - you could re-map your users or just re-assign those combinations of groups and permissions to suit what you need. And maybe rename the current 'guests' group. The options there are up to you...
