Sorry, not following you... What do you mean by two rows of member types?? that image looks okay to me... are you asking about the table headings at the top and bottom? those are standard wp way of doing tables with multiple action possibilities...
subscriptions have nothing to do with mapping of wp roles to sp user groups...
when a user registers on your site, they will get a wp role assigned to them... on forum - usergroups - map users to usergroups, you specif how you want to map those wp roles to sp user groups... so on that panel, you will see all the wp roles listed... you need to map each role to the sp user group you want that user to get when they register on your site... after setting them, click on the Update Mapping Settings button... note, this will only apply to new users...
if you want to map all existing users to an sp usergroup, then you would be sure to update the top section (as explained above) and then pick your settings in the bottom section of that panel... you can either add memberships based on roles (any existing sp user group membership will remain and new ones will be addd) or you can replace any existing memberships with the usergroup based on their wp role (replaces)... the option you indicate above would add new membership (it will not duplicate)...
but it all starts with having the wp roles to sp user groups mapped first...
