Hello, Everything on my forum is setup and working well right now except that nobody is able to upload an avatar from their computer. Below is all my settings to hopefully fix the issue as soon as possible.
I have enabled avatar uploading and selected to display avatars under the Profile panel. I went ahead and set all Permission Sets to enable avatar uploading incase that was an issue. I have checked the Storage Location to see that the folder forum-avatars is checked and ok to write. The WP setting is enabled to allow users to create accounts. The options for users avatar is set to Upload from PC, Gravatar, then SP Default. I have essentially left the 3 User Groups to be Guest (Read only), Members (Standard Access), and Moderators (Mod Access).
The avatar upload settings are at 50 pixels wide and 280000 bytes, allowing resize at 90%. I can browse to select the image but when I click Upload Avatar nothing happens and on a double click I get the message "Sorry, the avatar file already exists". Any ideas as to what might be causing the issue? Thank you!

Is this with any browser or just one specific one?
and not sure I follow the steps, so lets walk through it... you can click on browse and get a popup window? you are then able to browse your computer and find the image you want? and when you click open, the popup closes and the avatar filename is inserted into the field? and the problem you refer to is after all that when you click on upload avatar? am I interpreting your words correctly?
check your avatar storage location... does a file by the name you are trying to upload exist?
do you have a link to site to check for obvious js issues?

Thank you for the quick response. I have tried on Chrome, Safari, and Firefox and the issue is still there.
You are correct, I can click Browse, search the popup for the image, select it, and see it listed on the SP page, but once I click Upload Avatar nothing happens. If I double-click Upload Avatar then I receive the "Sorry, the avatar file already exists" message.
I will work to double-check if any files other than default SP avatars are in the forum-avatar folder. I will also PM you with the site as it has only been shown to test users at this point and should remain confidential until then.

For me too, impossible to upload some avatar ...
I got this :
Files accepted: GIF, PNG, JPG and JPEG
Maximum width displayed: 50 pixels
Maximum filesize: 10240 bytes
ONLY 10240 bytes .... Is it normal please?

Ludovic, yes 10k is the default size for avatar - should be big enough... too big an avatar will hurt site performance.. but you can make it whatever limit you want on forum - profiles - avatars... not sure what you 'got'... you could attach a screenshot...

Just an update to the issue. I was using WP 3.4 and needed to upgrade to 3.5. The issue has been resolved and everything seems to be working properly now. Thanks Mr Papa for the help!

Great news! Thanks for the update... Again, apologies for slipping in a wp function that appeared in 3.5 and not updating or min required version...
