Overview:
We are having a problem with our forum where the forum itself is repeating.

The Story:
We recently upgraded our live site (above) using the following steps.
1) Last month we copied the live site to dev.
2) We upgraded the dev site to the most current version of wordpress and the forum
3) Today we moved the upgraded dev site to live
4) I then did a manual upgrade of the forum on the live site which worked fine.
5) We expected this upgrade of the files and database would have resolved the problem with the forum showing up multiple times since all the files changed.
The question is,
a) do you have any idea where can look for this problem?
b) do you have someone that can look at our files if need be?
When I select "WP list pages", it too, will bring up "forum" underneath each of my page tabs (multiple times). If you click on these new forum buttons, it will take you to that page that the forum button is sitting underneath.
Deselecting WP list pages eliminates this problem and I only have my page titles showing at the top (not "forum" underneath each page tab). When I select the forum tab and it pulls up that page, the "forum" tab no longer appears at the top. The remaining tabs all appear though. Everything else is fine....I can use the forum, navigate to other pages, etc.
My question is....is this by design? Or is this something random. It's not a huge deal...more cosmetic. I tried selecting all the other combinations of the Integration Options and Theme Display options and found that the only thing that makes a difference is the Filter WP list pages. But if I select the filter WP list pages, the forum page title remains at the top (but with the repeated forum buttons below).

some themes and plugins do wonky things with the_content filter... try going to forum - integration - page and permalink and play with the display multiple and display in loop options... not sure I follow what you did there...
did you also try the strict wp api option?

I actually tried that: went to page and permalink and tried pretty much every combo possible. The only thing I could determine is that selecting Filter WP list pages will bring up "forum" repeatedly. All the other options did not make a difference. So, when I deselect Filter WP pages, I no longer get the repeat. However, when one goes to my forum page, the actual page tab that reads "forum" disappears. I removed all my plugins, too to no avail. Maybe it's the theme.....but since it's not that critical, it's probably not worth the time.
One last thing on this topic, though... I receive an error on the Page and Permalink that reads "The page slug is either missing or incorrect. The forum will not display until this is corrected." Reading the help icon, it appears that this slug not loading is not out of the ordinary. I simply selected from the drop down and chose my WP page to attach it to - as instructed. But this error warning never goes away and I am able to access/display the forum. I just want to make sure that this is constant error notice is normal ....

First up, the filter wp_list_pages option in SP is an attempt to work around a very old bug in WP that they seem reluctant to fix. It goes back years! What you are experiencing is probably a combination of that, our workaround and the way your WP theme is creating and displaying the menus. Every now and then - rarely these day though - we come across a new variant of menu display that we have not catered for in the workaround which throws it off. I reckon this is probably such a case. Given your WP theme and time to install it and work with it then we could probably fix it.

No this is not normal and not desirable! Needs to be addressed.
Can you please do the following: Go to the WP admin > Pages > All Pages and bring up the list. Locate the page being used to display the forum and hover the mouse cursor over it. Down in the browser status bar you should see the url of that page being shown. Part of that will show 'post=XX' where XX is the ID of the post/page. What is that number please?

not entirely sure where Andy was going, so may need him to come back...
but is this 5.1.4? and is the page a top level page or perhaps a subpage (ie has a parent)?
