Yes, it is 5.1.4 and I have the forum on a top level page.
I only installed this a few days ago and it's been like this from day one.

Well we get these unique, one-off things come up every now and then but this really is a weird one. Are you noticing any other problems - specifically in the forum admin arena? Can you generally edit and save data OK?
And is there anything different or unusual about that WP page? Is it actually published for example?

The only other problems I have really noticed were the issues I had with the other plugins and using IE. When I disabled the plugins, the problems went away. So, I would suspect those are not related.
I have no problems in the admin either....I am able to edit and save.
The WP page is published, but the site is not live yet (not sure if that has anything to do with it). Other outside users have been able to access and use the forum.
If you think it could help, I don't have a problem uninstalling and reinstalling.

Oh dear. I was really hoping for something unusual in the slug but you can't get more basic that that!
Do you by any chance know you way around the database? Are you able to look at the tables? This has me completely stumped at the moment I have to say so knowing some data values as things are changed could be useful.

Unfortunately, I do not know my way around the database. Is this something where I could give you permissions into my site or would you be able to give me some directions?
Maybe I will just try uninstalling it and then reinstalling it and see if that makes a difference......

Re-installing shouldn't really make any difference to anything in theory. And if everything else is working Ok then probably not worth it. It is just a puzzle to be solved.
So - Steve - any ideas why the page slug would go missing when the integration form is opened?

Well, you were correct in that re-installing would not make a difference. Thought I would at least give it a try and I am still getting the error. One thing, however, is that I am not longer getting the repeat "forum" tab or it disappearing when I am on the forum page.
So far looks like the forum is still fully functional....
