Issue #1: Profile Options
The profile options options are my site redirect to a completely unformatted version of my index. I want the profile options to be flush with the rest of the site and forums. Through some googling, I've found that some folks have a check box to wrap the profile options with the header / footer, but I do not have that option any where. I've tried all of the different options in the Profiles > Profile Options > Profile Display Mode area, but none of them seem to do anything.
Issue #2: SQL Errors
We are using a lot of plug-ins, as most of them seem like features that come included in more other forum packages. I'm not entirely sure this is being caused by the forum itself or an individual plugin. Given that there are 45 of them, going through one by one to check is not reasonable. Similarly, there is no function to 'select all' for activation / deactivation, so just turning them all off and on takes more data entry than it should.
How can I tell what is causing these errors?
Profile:
There is no such option. That option goes back to a very old version. We have covered this in email and I note from your last email that you were saying you are indeed getting a 'jQuery not defined' error in the console. It is quite simple. The profile is being directed at a blank page because the AJAX call is not able to to run. This is still likely to be caused by an issue such as that described in the codex article we linked you to.
SQL Errors:
Just so we are clear - we have stated many, many times why we devolved functionality to plugins. It allows for a lighter, cleaner and faster 'core' and enables users to only select and include those features they want instead of being saddled with them all. This makes sense on so many levels and has worked beyond our expectations.
The errors there are also not in SP plugins as can quite clearly be seen by the path being reported as the site of the error.
The errors you are seeing are also not SQL errors. These are actually PHP errors. For some bizarre reason it looks like your host may have blocked access to the PHP MySQL functions as these are being denied and thus causing the error. If this is the case then it is the first time we have encountered this. It would be useful - for starters and before we determine how to deal with this rather unique circumstance - if you could get confirmation from them on the position with these PHP function calls.

i'll investigate about the php/sql errors after contacting our host and reply back.
I must be missing something in that codex. So far, aside from linking to JS from our domain rather than google or any other, all I've gathered is it "must be something with our theme or other plug ins". Although I dont doubt the truth in that statement, it gives me almost nothing to troubleshoot with...
If you DO have a badly loaded jQuery or jQuery UI (and I do not know this as I have not seen your web site) then there is not much we can often do regarding identifying where it is being loaded from - unless explicitly from a WP theme or plugin folder - ion which case just looking at the generated html source will quickly show that.
