OK - so we ARE talking about using the Admin Bar plugin - which has the 'View' option and the buttons to delete, approve, go to topic etc.
So - the most curious thing here is that you say 'one of your moderators' sees this phantom post. But the admin bar is a shared resource - in other words all moderators/admins that can use it will see the same results (although moderators will be subject to their permissions set up).
So what about say, the SP admin. Are they seeing this post?

Oh sorry, no admin bar. I mean when he clicks on the link in the moderation announcement at the top to the deleted post, it goes to nowhere and the moderation prompt is still shown. I've switched him between various user groups and it still shows at the top apparently.
Ah OK. So we still have the odd minor issue with removing a notice if the post gets moved or deleted. I will open a ticket and have another look at that to see if I can locate the issue. Thanks.
This notice is actually about posts made by that user. As we hide posts awaiting moderation then there is no way for, say, an unregistered guest user to ever see that the post was properly saved or to check on it's status. Hence the notification. So either this now removed post was originally made by your moderator (perhaps when not logged in if you allow guest posting) - or it has somehow become cross lined top the wrong user.
It will disappear after a short while as it does have a lifespan. In the meantime - if you wanted to remove it manually then you would be able to locate it in the 'sfnotices' table and delete the row there.
I will see what I can find in the code. Do you by any chance remember how this post was dealt with originally?

It was during testing and he forgot to tell me (/sigh :P). He created the post at which I created a newly registered user to check the permissions. With the newly registered user I posted a new post which prompted with a moderation alert. I then changed the permissions and privileges for the usergroup for the test user and deleted the rest of the topic. The test user was also later deleted from users list.
What's the duration of the alert until it disappears?
