I apologize I didn't chime in on this earlier, I've been sick - our server times out when trying to upload large files, and I think it's when this happens that the "image already exists" error happens (when they try to upload again).  I've increased memory but it is still timing out on occasion.  I will test the fix when I get out of meetings this afternoon.  Thanks again.
