i think I can only come up one scenario that would cause an intermittent issue like this. That would be if, after loading the page but prior to the user making a post, they used or performed some other activity on the site that threw a scripting error which would then possibly stop other scripts from running.

I am not for one moment blaming this but a good example of what I mean would be the user loads the topic page, uses the chat box on the right and then writes a post. if the chat code creates an error then the captcha script could well just stop responding.

The obvious things to do would be to (1) ask the user of they did anything like this on the page before making the post and (2) trying the various other items on the page (like the chat) with the script console open to see if any errors are reported.
