Sorry... didn't see that question. 

I'm trying to remember. The user used to be an admin. I believe I found him in the Dashboard > Users > Administrators list and deleted him from there.

If deleted via the WP Admin - Users panel then yes - it should have removed itself from the forum members list. Do you have many of these? Would it be appropriate to remove this one (Try the forum admin > Users list where there is a delete option? And then monitor to see if this happens again?

As for the permissions issue, I reset again... then added permissions for each topic. It seems to have worked.

My apologies. I'm overworked and exhausted in addition to not being very technical.
