I think the biggest problem is that you are serving cached forum pages.. so things wont work... you cannot cache the wp page the forum is on since its dynamic in nature (all the content appears on that single wp page)...
for each type of caching you are using, you need to tell it to ignore the wp page the forum is on... basically in rejected strings add /forum/

I have prevented the forum page from being cached but am still experiencing the same issue.
I also tried deactivating W3 Total Cache completely, but that still doesn't solve the problem.
Any other ideas or avenues you think are worth me investigating based on your expertise? I have been trying to get this issue figured out for over a year now. I'm not sure where to turn next! lol

Eureka! Just got it to work.
The issue was the file size limit. I had the limit set at 10,240 and simply needed to bump it up a bit.
I think my problem is that I was not receiving any notification that the file size was too large. Said another way, I tried uploading the "too big" photo and never received an error message or notification that the file size was too large.
Either way it seems to be working OK now. Thanks for all the help AND patience.

Unfortunately I am not seeing the popup. I think there are still some quirks with how the uploader is working on my site.
I'm happy for now just to have it somewhat working, and thanks so much for the support. Huge help believe me.
Right now I don't get that quick popup message if the size of the photo is too big.
Also, after uploading a photo, a user cannot see that the photo indeed uploaded, until they navigate back to the forum. Said another way, they upload the photo and they do not immediately see that the photo has uploaded, until they navigate back to the forum and then it appears.
Just a couple more quirks I have on my To-Do List.
No need to spend any time on this for now, unless you happen to already know a fix. I'll log some of my own time in and then check back here down the road if I need assistance.

leaving aside the notification angle for now - you seem to be saying that users can uploads photos but not avatars? Is that correct? Which is odd as the same technology is used for both.
Are you seeing anything in your php error log after trying an avatar upload? (forum admin toolbox > error log firstly and then your server based error log).
