It turns out the culprit was this plugin http://wordpress.org/plugins/a.....r-manager/
Have uninstalled it and moved to an alternative, my users will have to re-up their avatars though.
In saying that, my avatar priorities are currently:
- WP Avatars
- Uploaded Avatar
- Gravatars
- SP Default Avatars
- Avatar Pool
- Remote Avatar
However, I'm getting default WP avatars even for users who have uploaded one in the past, is this because the WP default avatar is counted so it doesn't try to pull the uploaded avatar?
Hope this makes sense!

Yes - the problem with that priority sequence is that one way or another there will ALWAYS be a WP avatar so it will never try beyond the first option. If you want WP avatars to be the default if the user has not uploaded one of their own, then you will need to switch those first two around.
