This is 99% likely to be a javascript conflict. the image in post 4 above would suggest that by that time jQuery was already broken. It tends to error when the next attempt to use it occurs.

is there any chance you could deactivate all other plugins that use any javascript to see if the problem resolves itself and, if so, reactivate them one at a time trying between each one until it breaks again? You would need to clear your browser cache between each attempt as well.

Kinda off topic but I thought I would try and see what is going on live on your site.

I registered and then immediately saw this on Thank you for Signing up page.

All plugins in WP and also all plugins in SP were deactivated except File Uploader plugin in SP. Unfortunately it still doesn't work.

If what Steve wrote in post 12 above is the case, and I replied to that topic, then it actually looks more likely to be the theme - possibly in concert with a plugin. Those are serious site wide problems, not just for SP.

It was tested with same process in post 16 using Firefox. It doesn't work, either.

Well if there is no other javascript conflicting with the code then the inly other thing that comes to mind is a corrupted install although this is extremely doubtful.
Have you looked at the error console or tried it with Firebug to see what is reported?
