I doubt its related... but something (and not likely wp) is adding to it...  there is even some w3 total cache stuff in there...

I totally agree, something else is adding these values to htaccess.  I am not using W3Total Cache.  I stopped  using it a long time ago.  It used to mess up the sidebar login and also created conflicts with some other plugins.  It was getting late at the east coast.  I will clean it up today morning.  Most of the entries in the htaccess are for denying IP access and spam bots from well known East European, Chinese and African scammers.

I have replaced the htaccess file with a basic htaccess file.  The issues remain same.

The Who's online plugin cannot be activated anymore.  It gives 403 Permission Denied, even wiith new basic htaccess file.  The permission on the directories are correct.  I recked the permissions.

given the multitude of layers of checks you are making with .htaccess and plugins, is there any chance you are running mod_security on your site? Its a crappy addon for apache... Also a 403 is a message coming from your server that its rejecting the request... Something we have seen with mod_security or something called Bad Behaviour... both are known to have some false positive issues... Are you running Bad Behaviour?

Good catch.  I am not running mode_security, but I am using Bad behavior.  Let me disable it.

Bad_behavior disabeling did take care of the online plugin activation.  The other issues remain.
