Good eye catching the hyphen versus underline.
I'm turning off other plugins to see if that has any effect. I do notice some errors in the SP Toolbox Error Log on my site. Does anything you see there help in identifying the problem?

if you have any genuine errors in the error log then it might be worth pasting them here. Notices and Warnings are unlikely to yield anything useful...

Good news! I discovered what appears to be the source of the 404 error. The Types plugin that I use appears to be causing the problem. I recently installed a development version of this plugin to resolve an unrelated issue. See below my post to the Types support forum at http://wp-types.com/forums/top.....ost-244116.

I believe I found a problem in the Types 1.6.2 development version. The problem manifested in another plugin that I use, Simple Press. The problem is that a filename such as "sp_Profile.png" is being changed to "sp-Profile.png" in the HTML when the page is displayed, resulting in a 404 error on that image file. If I disable the Types plugin, the problem goes away. When I enable the Types plugin, the problem returns. Can you confirm my finding and work a fix into the next release?

Wow OK... Well great news that you found the culprit. I'm not sure if the error logs will be of any use if they are caused by the plugin being active. Hope the developer of the plugin can resolve the problem..

Developers of the Types plugin have confirmed the problem and promised a fix. Troubleshooting this was a real eye opener for me. The page that displays the SP forum doesn't make any use of the Types plugin, yet the Types plugin manifested the problem on the SP forum page. A good lesson in why it's important to disable other plugins when troubleshooting, even those plugins that appear to have no relationship to the problem.
Kudos to Yellow Swordfish for spotting the underscore to hyphen change in the filename.
As far as my SP error log goes, should I do anything to address the (possibly unrelated) problem(s) that produced the log entries?

Good to hear you are on the verge of getting a fix for this.
You can clean out your error log if you like;. The Duplicate key on Group in the sfcache table is something we are aware of but so far we have been scratching our heads over as there is no way it should actually happen! But hopefully we will discover the problem before long and fix it up. There should be no ill effect...
