Scott, I have not seen a reply to Andy's request (did I miss it?) for you to check forum - integration - storage locations and make sure that its okay for storage... I see your server permissions, but want to verify its web accessible by us...
If that is good, then perhaps you could provide a temp account via pm to yellow swordfish and myself with sp admin caps so we can see whats going and look at the ajax response...

taking a look... quite odd... thinks its saved and in fact something is evident by the remove being shown... but should be the image...
now, those images are huge! 420x420... I would suggest something more like 32x32 but definitely smaller... we dont adjust the size, because users are free to use what size they want - some go smaller than our 32x32 and some go slightly larger... I dont believe that's related to the problem, but when displayed it will look most odd at that size...
can you look at your db for me? for the twitter forum, please look in the sfforums table and then the twitter forum row... is there anything listed in the forum_icon column?

Thanks Mr. Papa and Yellow Swordfish, I have got it working now.
It appears that Mr. Papa was right and images were far too large to be accepted. I have reduced them from 420 x 420 to 33x33 and it works perfectly now => http://lowcostmarketingstrateg.....com/forum/
