And, I suppose it is not unexpected, but it is working fine for me using the latest versions of everything including Unified. This is testing on Mac OSX under Firefox. It also has to be said that we have no other reports of a problem with the captcha plugin either.
I note also that the display of the captcha icons is somewhat off on your image. They should be horizontal not vertical like that.
So - the most usual thing in these sort of unique circumstances is a JavaScript conflict between the captcha script and something else you have active on your website. The only way to try and discover this is to have the captcha plugin active - open the script console marked to just show errors and then try to run the captcha and see what gets reported. Is this going to be possible?

Did you try the captcha in the WP registration form? That's the one that is broken (as my initial screenshot showed).
In the post form the captcha works fine, also for me. It's the WP registration/signup that is broken. There are no Javascript errors.
In case it matters, I'm using the stock Twenty Fourteen theme in WP.
I am sorry - I completely missed that!
Seems like the CSS is not getting loaded so i am going to have to hunt down why that is. I will get a ticket opened right away and investigate. Sorry about that - odd it has not come up before this.
I would suggest, of course, turning off the use of the captcha on the registration form until we have this one nailed.
