I think many people – and certainly we do here – prefer, when they are following a topic, to know that there IS a post from somebody and it just awaits approval to be seen. And, of course, as long as they can not see the content – it is harmless.
There is no quick and simple answer to this. But you could write a filter against the topic query to not include all posts in moderation. That would work up to a point and you could exclude that happening for admins and moderators.
There may be a few anomalies though. The post index number will miss one of course, If it is the first post in a new topic (not uncommon) the topic might be loadable but there will be nothing shown. Not sure about that without trying it! And if someone copies and uses a post permalink it may be completely wrong after the post is moderated if the post in question is on a page boundary. Maybe others. I would need to try.
