Member Add failed?

hi friends, one of my site admins shows up in the list of "Members not belonging to any usergroup". I want her to be a Moderator in the forum but when I try to add her to the Moderators, I get the message "Member Add failed". 

What's the solution, please?

please detail the steps you took to add them as a moderator... some screenshots might help too... just want to make sure all the steps were accomplished in the add member dialog...
