It says the location is found and is writable, same as all of my other folders.
I noticed there is a forum identities folder in sp resources just for the images and there is the plug-in folder itself in the sp plugins section. Should the forum identities images folder be in the identities plug-in folder?
We can sort one out for you but I don't believe it will make any difference. I have used the latest without problem and have been working with a user just yesterday who had no problems with it.
The image upload routines that are used here are pretty standard and used in all other backend uploads (and Avatar on the front end), That is why it is worth investigating the server side of things which seem to be the more likely source of the issue.
I realise you have a write and read tick mark against the folder but could you check with your host if there is any reason why this can not be written to?
And just perhaps, in case this is a weird issue, to at least discount it - try with an icon that has a filename composed only of standard characters - no spaces.

Thank you Yellow Swordfish! Changing the file name of the picture to one with no spaces and only standard characters worked!
However now I have another problem.
I've set up the special template function. The icon displays where I want it. I entered the address of my user page for this particular site in the proper identities slot on the edit profile section.
However when I click on the icon, it goes straight to the homepage of the base url I entered in the plug-in's admin page, not my user page as the default identities would do.
How can I fix it so that when I click on the icon it goes straight to my user homepage and not the base url?

can you show us example of your base url and then the fragment entered in the profile? and the url you are sent to...
