I have 1and1 hosting on a dedicated, managed server. I looked in the Logs directory and can see lots of files, but don't know what the error log file name would be. There is one file called error.log but it was last modified in May 2013.

Permissions on wp-content are 755, sp-resources are 705, all folders inside are also 705, however within forum-plugins all directories (including subscriptions) are 755.

Within subscriptions, all directories are 755, however the php files are 644.

Would 705 or 644 be a problem? I can use Filezilla to set everything to 755.

I can give you FTP access if it would help troubleshooting.

Idea: Would it be useful to try adding some code to the page to manually load the missing CSS file?

705?  quite odd...  never seen such a setting... doesnt mean its wrong, just I dont understand it...

yes, please make sp-resources 755 as well as all the subdirs inside it..  php files should be fine at 644...
